Frozen shoulder can make ordinary movements surprisingly difficult. Reaching for a shelf, putting on a jacket, fastening a seatbelt, or finding a comfortable sleeping position may become painful and frustrating. Before long, even washing your hair can require careful planning.

For many people, the condition eventually improves with physical therapy, stretching, medication, or injections. The difficult part is that improvement can be slow. Some patients continue to live with significant pain and stiffness despite months of treatment.

When pain and stiffness continue despite conservative care, shoulder embolization may be worth considering. This minimally invasive shoulder pain treatment targets abnormal blood vessels associated with ongoing inflammation around the shoulder. It is intended for select patients whose symptoms have not improved enough with more conservative care.

Why Does a Shoulder Become “Frozen”?

Frozen shoulder is the common name for adhesive capsulitis. It develops when the capsule of connective tissue surrounding the shoulder joint becomes inflamed, thick, and tight.

A healthy shoulder capsule is flexible enough to let the arm rotate and move freely. When the capsule stiffens, that movement becomes restricted. You may have trouble lifting your arm on your own, and someone examining your shoulder may also be unable to move it through its normal range.

Frozen shoulder usually progresses through three stages. During the first stage, often called the freezing stage, pain increases as movement begins to decline. Nighttime pain can be especially disruptive. In the frozen stage, the pain may become less intense, but stiffness remains. The final thawing stage is when movement gradually begins to return.

The timeline is different for every patient. According to the American Academy of Orthopaedic Surgeons, complete recovery can take as long as three years.

Frozen shoulder is most common in adults between 40 and 60. People with diabetes or thyroid conditions have a higher risk, as do those whose shoulder has been immobilized following an injury or surgery. Sometimes, however, the condition develops without a clear explanation.

What Is the New Treatment for Frozen Shoulder?

Shoulder embolization, also known as shoulder artery embolization or transcatheter arterial embolization, is an emerging treatment for persistent frozen shoulder as well as rotator cuff-related pain, osteoarthritis (OA), and chronic inflammatory pain due to conditions like synovitis and bursitis.

Researchers have found that chronic inflammation around the shoulder may be associated with the growth of small, abnormal blood vessels. These vessels may help sustain inflammation and can be accompanied by pain-sensitive nerve fibers. Together, they may help keep the cycle of pain and inflammation active.

Shoulder embolization is designed to reduce blood flow through these abnormal vessels. It does not remove part of the joint, repair a tendon, or cut through the shoulder capsule. Instead, it addresses one of the processes that may be contributing to persistent inflammation.

This treatment is generally considered after physical therapy, medication, injections, or other nonsurgical approaches have not provided enough relief. It is not the right treatment for every painful shoulder, which is why an accurate diagnosis must come first.

How Is Shoulder Embolization Performed?

The procedure begins through a small arterial access point, usually near the wrist or upper leg. Through that access point, the physician inserts a thin catheter and guides it toward the shoulder using real-time imaging.

Contrast material helps the physician see the shoulder’s blood supply and identify vessels connected to the inflamed area. Once the appropriate vessels have been located, a small amount of embolic material is delivered through the catheter to reduce abnormal blood flow.

The physician then removes the catheter and covers the access point with a small bandage. There is no large incision, and the shoulder joint itself is not opened.

At Vascular Institute of Michigan, shoulder embolization is typically performed as an outpatient procedure with local anesthesia. Patients undergoing this treatment generally return home the same day, and the procedure typically involves less initial downtime than shoulder surgery.

Does Embolization Actually “Unfreeze” the Shoulder?

Not directly. Shoulder embolization does not force the joint to move or mechanically loosen the tight capsule.

Its role is to reduce the inflammation-related blood flow that may be contributing to pain. If the shoulder becomes less painful, it may be easier for a patient to stretch, participate in physical therapy, and gradually rebuild range of motion.

This is an important distinction. Shoulder embolization is not necessarily a substitute for rehabilitation. For many patients, the procedure and physical therapy may work together. Embolization addresses pain and inflammation, while therapy focuses on restoring movement.

What Does the Research Show?

Research into shoulder embolization is still developing, but several studies have reported encouraging results among patients with adhesive capsulitis that did not respond to conventional treatment.

A 2025 study published in CardioVascular and Interventional Radiology reviewed 128 procedures performed in 118 patients. Six months after treatment, researchers reported significant reductions in pain as well as improvements in shoulder flexion and abduction. Clinical improvement was documented after 115 of 128 procedures, or 89.8%, at the 12-month assessment. No major adverse events were reported.

An earlier pilot study available through the National Library of Medicine also found reductions in nighttime pain and improvements in shoulder function following embolization.

Who Might Benefit From Shoulder Embolization?

Shoulder embolization may be worth discussing if frozen shoulder has continued for several months and is interfering with sleep, work, or daily activities. Many potential candidates have already tried physical therapy, anti-inflammatory medication, or injections without lasting improvement.

A physician will also want to know how much movement you have lost, whether your pain is connected to inflammation, and whether another condition is responsible for your symptoms.

Not all shoulder pain comes from frozen shoulder. Similar symptoms can occur with:

  • Rotator cuff injuries
  • Shoulder arthritis
  • Tendon inflammation
  • Shoulder impingement
  • Bursitis
  • Nerve problems
  • Previous fractures or other injuries

Shoulder embolization cannot repair a torn tendon, stabilize an unstable joint, or correct significant structural damage. If one of those problems is present, another form of shoulder pain treatment may make more sense.

What Are the Potential Advantages of Shoulder Embolization?

One of the main advantages of shoulder embolization is that it does not require open surgery. The procedure uses a tiny catheter entry point and is generally completed without general anesthesia.

It also allows patients to return home the same day and usually involves less initial downtime than a surgical procedure. Because the treatment targets selected blood vessels, healthy surrounding tissues can be preserved.

The goal is not simply to make the shoulder feel better for a few hours. By reducing a source of persistent inflammation, the procedure may provide enough pain relief for patients to sleep more comfortably, use the arm more naturally, and make progress with rehabilitation.

Results still vary. Some patients may experience meaningful improvement, while others may have limited or gradual relief.

What Are the Risks of Shoulder Embolization?

Minimally invasive does not mean risk-free. Possible risks include bruising or bleeding at the catheter entry site, temporary discomfort, infection, blood vessel injury, contrast reaction, skin changes, or unintended blockage of a nearby blood vessel. Contrast material may also require special consideration for people with reduced kidney function.

Careful imaging and precise catheter placement are used to reduce these risks. Before treatment, the physician should review your health history, allergies, medications, kidney function, and any conditions that could affect circulation or healing.

What Is Recovery Like After Shoulder Embolization?

Most patients go home after a short period of observation. Some soreness or bruising may develop near the catheter entry site, and the shoulder can feel temporarily uncomfortable following treatment.

Strenuous activity is usually limited for a short time, although the exact instructions vary. Your physician will explain when you can return to work, exercise, physical therapy, and other activities.

Improvement is often gradual rather than immediate. Some patients begin to notice less pain within a few weeks. For others, changes develop over several months as inflammation settles and movement becomes easier.

A physical therapy or home exercise program may still be recommended. Reducing pain can create an opportunity to move the shoulder more comfortably, but consistent stretching may be necessary to address the stiffness that has already developed.

Is shoulder embolization considered surgery?

No. It is a catheter-based, image-guided procedure performed through a small access point. The physician does not make a large incision or surgically open the shoulder joint.

Can shoulder embolization help me avoid shoulder surgery?

It may provide a nonsurgical option for some patients whose pain is connected to chronic inflammation. It cannot replace surgery when a tendon needs to be repaired or another structural problem needs to be corrected.

How soon will I notice relief?
